Im getting an undefined variable error,

Notice Undefined variable: Last Name in c:\pathtomywebdir\include\business_logic.php on line 1388

ANy ideas on whats happening there?

Probably you have a field name that contains blank spaces, DaDaBIK doesn't handle them.
